Vomited 4 1/2 hours after yaz, where do I get the extra pill

I'm taking yaz and this morning I took it at the normal time (7:30am). I vomited at about 12:10pm. My fiancee and I had unprotected sex yesterday morning. Would the hormones have been absorbed after 4 1/2 hours? In the booklet I got about yaz it said that if vomiting occurs within 3-4 hours after taking the pill, it should be treated as a missed pill. When you miss a pill you are supposed to take it as soon as you remember but where do I get the extra pill from? Also, I only have 4 active pills left before the sugar pills so should I skip the sugar pills this time and move straight onto the new packet? The only issue I have with this is that I won't get the withdrawal bleed so I won't really know whether I'm pregnant or not. I guess I could always take a pregnancy test...
I know I should have got all of this info from my doctor but he was really unhelpful. I have been to him twice and I'm definitely not going back again. The only problem is that I can't find another doctor who is taking new patients at the moment.

Testing has shown that the pill absorbs in about 2 hours, so you should be safe. It is guaranteed that the manufacturer built substantial safety margins in the 3 to 4 hour recommended time frame.

Continue normally with your birth control pill regimen.
