58 yr old woman experiencing swelling and redness in right leg, down calf and behind knee, particularly after long shifts of standing during work. Wears wrap on knee to prevent pain.

I appreciate your concern

The leg needs to be examined look for significant swelling warmth and tenderness. Doppler ultrasound of the lower limbs should be done if these findings are there on the physical examination to look for a possible deep vein thrombosis. Acute gouty arthritis can also present with these symptoms. If there is significant knee joint swelling then joint fluid must be aspirated to rule out the possibility of septic arthritis. If on clinical examination the patient has had this problem for a long time , the patient is obese and has been using and nsaids and prior diagnosis of osteoarthritis is also present in the records then symptomatic treatment can be advised. Counseling regarding knee replacement therapy should also be done.
