My right breast is sore pretty often. The nipple gets this weird shape to it like dome but will go back to it original shape. Should I be concerned about this?

Welcome to ehealthforum,
The breast soreness along with change in nipple shape could indicate serious issues. Look out for lumps in breasts. Look for changes in appearance of skin over it. If there are no lumps present, then the problem could be attributed to hormonal issues (but here too, both sides should be equally affected).
I would recommend you to go ahead with a detailed examination done by a gynecologist/healthcare provider to look for lumps in breast and get screening for breast cancer done.
