Welcome to e health forum.

From the history given, chain smoking and long history of Smoking (40 years) and the current symptoms, it is indicative that your father might be having Chronic Obstructive Pulmonary disease (COPD).

COPD is a progressive lung disease, which worsens with time. The condition is caused by damage to lung tissue as a result of long term irritation of the tissues due to smoking.

Such damage is associated with destruction, inflammation and thickening of the lung tissues, with resultant loss of elasticity and reduced function of the lungs. Hence this results in reduced oxygenation of the blood that flows into the lungs, and resultant symptoms like generalised weakness, cough, difficulty breathing, etc.

Due to the slow progress of the disease patients get diagnosed only in late stages, when the condition becomes irreversible. The only treatment in this condition would be with Oxygen therapy, medications to help more air entry into the lungs (bronchodilators) and antiinflammatory medications (to control the progress of the condition).

The condition worsens with seasonal changes and respiratory infections which may necessitate urgent hospital care, antibiotic prescriptions , etc.

The way ahead, would be to consult with the current treating doctors or a Pulmonologist who can provide proper medical advice and counselling.

I hope this helps.
