some tripes using herbal extract pouring into ear for treating migraine pain. what will happen now? does the extract reach gut or it react with certain nerves directly in between?

Pouring anything in ear is not advisable unless recommended by a doctor. Anything that you pour in ear, acts locally. Some part may be absorbed by the blood and reach other body parts, but the amount is very negligible. So local ear drops etc are designed in such a way that they act within the ear only.
