hands and soles of my feet itch insanely, I then get hives

SEVERE ALLERGIC REACTION
Over the past few months I have been tested for allergies and diseases by an allergist/immune system specialist. The reason for these tests is because for the past 5 months, about once a month (not menstrual cycle related as it occurs at different times throughout my cycle)where towards the end of the evening I get very very bad heartburn and acid reflex. Then, at anywhere from 4:30am to 7am I wake up from my sleep with indigestion. Then th itching start. The palms of my hands and soles of my feet itch insanely, I then get hives all over my body (mostly on the thighs) and worst of all i get stomach pains that feel identical to menstrual cramps which hurt all the way into my lower back. The pain is unbearable and I have to nearly overdose myself on pain pills and noninflammatories to stay breathing. The pain is so bad I hyperventilate and struggle to breathe. The last attack has happened recently (I am 19 year old female) and I had the heart burn in the evening and woke up to the back/stomach pains without the itching and hives. I take a general allergy pill every single day so I think that prevented the hives and itching but the stomach/back pain still occurred and lasted for a grueling hour and ten minutes. I carry an epi-pen at all times but have not gone into full prophylaxis. I have been tested for allergies (no food or environmental allergies) and diseases such as systemic mastocytosis and lupus (all negative). What's wrong?!
