creatinine is high for the last year

what does it mean when my blood test show creatinine is high for the last year and now the estimated GFR is 54.

Creatinine is a protein your muscles naturally shed as they regrow new muscle cells. Your kidneys filter out that excess creatinine. If your creatinine is high (>3) it means your kidneys are not filtering it out so well.
Your GFR is really good (GFR of 5 to 15 is total failure) But the dichotomy of high creatinine and good GFR is not surprising. Your kidney's different filtering functions have a disconnect. They can be good at filtering out phosphorus but not good at filtering out excess fluid.
You definitely have some problems going on. You need to see a Nephrologist on a regular basis if you're not already and please please follow his/her instructions to the letter. That's super important.

P.S. to answer your question, it means you probably will eventually need a transplant, but it's such a case-by-case basis, no one could tell you exactly when, a month, a year, ten years...
