Good day Madam!
Thank you for asking. You have multi dimensions of troubles. Now the recurrent aphthous ulcers and oral discomfort history is explained by Tobacco use of 20 years. Seek a Dentist for the issue to sort out the aetiology for you. Increased ESR with tendinitis and raised C reactive proteins and GERD history All needs a little work up to sort out a an autoimmune aetiology like Lupus Etc or Behcet's.
Now the most Important issue is recurrent aphthous ulcers with joint involvement and Gi manifestation and the dots ned to be connected and i think a rheumatologist would have been a better option than an endocrinologist .
You have multi dimensional history and it needs a team to manage. So get one of professionals.
Hope it helps
Regards
