Q: Yellowish substance on tongue

For the past few months now, I've developed a whitish/yellowish substance on my tongue that is causing bad breath and a bad taste in my mouth. I brush, use mouth wash, and use a tongue scraper twice a day, yet every morning when I wake up the coating is back. Can anyone offer suggestions on how to permanently get rid of it? It just keeps coming back day after day.
