my penis head is very was normal , now is very wrinlky and my penis hole is messed up and looks like it has lips like the skin has been pulled back on my penis, has this been heard of, what do i do

please someone help me or guide me in the right direction , its pretty much like a layer of skin came off my penis, like it got pulled to hard.
