If it is fairly deep hole, and you really are beyond the dermis (all 5 layers of skin) the yellow sound like fat. Thats all, fat, We all have a layer of fat under the skin and it's not a always a "sheet layer".
Could be a cyst-doubt it unless it's hard, could be scar tissue.

Have it checked to be sure. You said you have had sex in over a year? Did you mean you 'haven't had sex in over a year'?
A blister like that could be just a pimple, but, not to scare you, is also a sign of syphillus. Though the lesion from syphillus is useally tender and hurts to touch. Also, after the lesions "pops", a hard scab forms.

It sounds as though it's an infected hair follicle. The subacious (sp) gland needs to be drained (popping it) or dried. Try the acne meds. It may take up to 14 days to fully clear.

See your reg doc 1st and have a hormone screen done. You may be a little high in estrogen. If not, It's normal for some men that they have large nipples and/or areolas.
