What kind of substance abuse was it exactly? Although substance abuse is damaging in the short-term all substance abuse doesn't necessarily result in permanent sexual problems.

You could try stretching it gradually using water-based lubricants. If you don't have any success with that, try consulting your doctor. There are prescription solutions or surgical treatments available for that condition if needed.
How long has this gone on for? It could just be minor scarring or a friction burn of some kind. Go see a doctor if the pain continues.
Blue tint? Do you mean the veins? If so, yes it's normal.

This is a guy's opinion but I think it's still useful:
A woman will probably think of penis size very similarly to how a man might think of breast size. Large breasts are, of course, attractive but essentially it isn't the size that matters. It's the shape and "presentation", if you will.
