My son age 15 have the following conditions :
1) alkaline urine (alkalinity is 9 tested with self help home kit)
2) urine turns pink or reddish after a few hours
3) occasionally have bad breath

He has blood and urine test a few months back and the doctor said is ok. But I am still very worry.

What could be the cause that turn his urine to turn pink or reddish after a few hours ?

A diet rich in citrus fruits, legumes, and vegetables raises the pH and produces urine that is more alkaline.
Bacterial infection of urinary tract can lead to more alkaline urine, too.
Urine color changes after few hours due to bacteria and atmospheric factors.
If his fresh urine is red or pink, than there can be a certain medical problem, like kidney stones, infection that lead to presence of red cells in urine.
Bad breath can result from dental diseases, frequent sinusitis, or problems with food digestion.
