Q: ultrasound of thyroid gland detected with nodule

Findings of ultrasound thyroid gland :
Both right and left lobes of thyroid gland demonstrate normal configureation and appearance, and are not significantly enlarged. Right lobe measures 4.8 x 1.6 x 1.4cm while left lobe measures 4.8 x 1.4 x 1.3 cm. Isthmus is not tickened.
There are 2 solid nodules demonstrated in the mid and lower poles of right lobe. Nodule in the mid-pole measuring 0.6 x 0.5 x 0.3cm, showing some small calcification within. The nodule in lower pole measures 0.6cm.
The palpable nodule in the right side of isthmus is demonstrated, it is ill-defined and measures 0.8 x 0.7 x 0.5cm.
6 small sold nodules or complicated cysts are present in the mid and lower poles of the left lobe of the thyroid gland which range from 0.2 to 0.5 cm in size. No dominant lesion is evident.

Dear Doctor,
Appreciate if you could advice if those nodules are benign or malignant. Are they worrisome or no attention is required? What are the chances that it is maligant? What would you recommend? Many thanks.

Unfortunately, the ultrasound results, as you have described them, can neither rule-in or rule-out thryoid cancer, as both benign and malignant thyroid nodules can appear the same on ultrasound, particularly when they are small (as in your case).

You have a couple of options as "initial steps:" One is to go on thryoid suppression therapy (by taking thyroid hormone tablets), and then undergo a repeat thyroid ultrasound in 3 to 6 months to reassess these nodules. Another option is to undergo an ultrasound-guided fine needle biopsy ("FNA") of the solid nodules.

You might ask to be referred to an Endocrinologist for further evaluation.
