Q: Treating phimosis with surgery

i have noticed for a while and have worried that my foreskin is really tight and when pulled back only a small amount of the penis head can be seen, to the point when doing sexual activities it hurts. why is this and what can be done, thank you.

According to your description (tight foreskin can’t be pulled back completely so the whole penis head can’t be seen and sex is painful) you might be experiencing phimosis.

Phimosis requires surgical treatment, so you may consult a urologist.
