Extremely eHealthy
I think that you need to take him to the er. If it's as bad as you say you should take him right away. He is facing dehydration. Please take him to the er. He can't wait a week. I cannot tell you what this is from as I am not a doctor. But please take him. How long has this been going on?

Go Immediately to the Emergency Room
Sometimes the fluid in the inner ear can cause vertigo like you are describing. I few years ago I had a boss that it happened to. He had exactly the same symptoms that you describe. I would not wait to see the doctor, I would go immediately to the emergency room.
