Q: Anxiety treatment, dizziness, and heart trouble
asked by: DoctorQuestion on December 11th, 2006
I have had anxiety for 2 years (probably longer but only diagnosed after the birth of my daughter, 2 and a half years ago) and during this time my symptoms where - chest pain, headaches, nausia, fatigue, moodiness, neck and shoulder aches, dry/tight throat, lightheadedness, loss of concerntration, breathlessness etc. Dizziness has been with me for the last 6 weeks. It is like an off balance/swaying feeling. I have read several posts and their are many people on this forum that suffer from this. I know what causes all my other symptoms and I don't seem to get over anxious about them now BUT this is horrible. Sometimes I feel like I am falling to one side or backwards etc. I have never passed out but I think that this is what is going to happen when I go through these feelings. I also get tightness in my face - under my eyes - like sinus pressure but my doctor reckons that this dizziness would not be from sinus infection etc. He has put it down to the anxiety
I have had all the test done and my health is good.

So my questions are:
1. What causes this dizziness?
2. I am parinoid about my heart rate - it is resting 60bpm and when I had a holter monitor once it show a sleeping rate of 33bpm at one stage, all the doctors I have been to assure me this is fine. Is it? Now when I get dizziness I quickly check my pulse to see if it is because my heart rate is low - it never is?
3. What is the best treatment for anxiety? CBT? Hypnotherapy? I only want a personal opinion on thisplease
4. Also I go months without taking my xanax .25mg and then sometimes I take 3 a day - is this normal with anxiety?
5. If it was a heart problem would xanax make the symptoms dissapear?

thank you very much for your time - this web site has helped me when I have had no where to go
Thanks

Anxiety and Stress Answer A1982
Anxiety disorders can manifest with many symptoms that can include symptoms like yours: chest pain, headaches, nausea, fatigue, moodiness, neck and shoulder aches, dry/tight throat, lightheadedness, loss of concentration, breathlessness, etc. Many of these symptoms can be found in real, organic diseases. That’s why anxiety disorder is also called "somatoform disorder". But before establishing this diagnosis, all the possible organic diseases of the lungs, heart, head, muscles, ears and eyes must first be excluded.

About your questions these are some concrete answers:

1. Dizziness can be due to inner ear disorder, cerebellum disorder, anemia, hypotension, etc. You can consult an ENT, neurologist and cardiologist before blaming anxiety for your dizziness;

2. Heart rate is normally lower during sleep because you are not doing any physical activity. I recommend not check your pulse during sleep again;

3. The best treatment for anxiety is psychotherapy combined with some of the new generation antidepressants like selective serotonin reuptake inhibitors (SSRI). You can consult a mental health professional about this;

4. Xanax (alprazolam) is a sedative and anti-anxiety drug from the group of benzodiazepines. It is used for temporary relief of the acute anxiety symptoms. Xanax can’t be used for long term treatment because it causes dependency and its effects become weaker in time;

5. If you have a real heart problem, Xanax won’t help you. But if anxiety causes you to think that you have a heart problem then Xanax can, most likely, help you.
