A friend was masterbating and found blood in his sperm.
and said it kinda hurt when he got off What could this be a sign of?

Blood in the sperm is oftern caused by prostate or seminal vesicle infection. I recommend that your friend visit an urologist. Sperm analyses will be probably be requested before prescribing antibiotics.
