First you should see an urologist to examine your “three” testicles. An ultrasound and spermogram must also be done. A “third”, less sensitive testicle may not be a testicle but a tumor. The quantity of sperm a count of spermatozoids present in your sperm hel determine the functionality of the testicles. You may need to see an endocrinologist specialist, too.
