Q: Serious Concern With Varicocele

I'm 22 years old and noticed a size change in my testicles. They had seemed to be significantly smaller than they use to be over a few weeks and I grew concerned. I was also experiencing on and off pain in my penis when I urinated, burning in my scortum, lower abodomen and low back pain, and "pre-cum" like discharge. I had an std test done and was negative. I had an ultrasound done on my scortum and revealed that I had a varicocele on my left side as well as mild hydrocele. No torsion or lesions were found at all.

Since I have realized that these symptoms were probably caused by the varicocele and that I will require either surgery or an embolization to correct this, I am concerned whether my testicles will return to their normal size. They were roughly around 2.5-2.75 inches a few months ago and during the ultrasound were about 1.75 inches. They have seemed smaller than they were after I had done the ultrasound (two weeks ago). My concerns are both about having children and being able to have sex, as I have a fear I am being slowly castrated by this varicocele. I have read testicles return to their normal size as blood flow is corrected from surgery or embolization but I have never heard by how much such as if your testicles are a 1/2 of the size they were, will they return to that size? My sex drive has dropped a bit and my testicles size change is rather noticable. Being my age you can see the concerns that I have. I have an appointment with the urologist in less than a week and would take a procedure as soon as possible. Anyone got any information they can share on this? This has been depressing me quite a bit the last little while.

Don't worry about it -- they will return to normal size once you have your surgery. Also, you may have some scrotal edema, which is excess liquid inside the scrotal sack. They can put you on medications to decrease the swelling after surgery.

What was the ultrasound like? Can you describe the way they did it? I've never done or had one, but am curious about it. Were you totally undressed? Did they shave the scrotum? How did they make sure the scrotum was loose and not constricted due to cold? Was it painful?

From what I understand from most people that will happen which is a bit of relief, although I had my appointment and was told the same when I went to the hospital about 2-3 weeks ago. Three doctors said they are of normal size and not to be concerned. Although they are not me which they understood, as I tried to explain that I had much large testicles. I was basically told that if it got worse to reschedule. They said surgery was only useful if it caused problems for me to have children or if I am in alot of pain. I was told surgery has risks although it is not major so they try to avoid it as much as possible. So right now I am extremely confused as what steps to take next. But the obvious I will be getting a second opinion asap.

The ultrasound basically had me lying down on my back. By luck I was not affected by the cold and they remained normal. I just had to wear a towel over it and had the jel on my scrotum. It was pretty similar to any other ultrasound I had before. There was no pain at all other than what I have from the varicocele at the time.
