Honestly, anytime you have sex, you are at risk for a pregnancy. It is unlikely, but just remember, even when you use protection, it can fail. Just be careful.

It depends what you mean by 'used a condom each time'. If you're talking about a new condom each time, then you're probably okay. Obviously you may wish to take extra precautions such as spermicides. If you mean the same condom, then that's a very very bad idea. Even though you can't see it, the walls of a condom wear down each time you use it, which is why they're meant to be used only once.
