Bacterial infection is a possibility, most likely, it's a fungus... Not sure why your doc didn't pick up on this, those are really the only two possibilities.

In either case, antibiotics will help if its bacterial.. If it's fungus you'll need to get the proper cream from your doc. You'll need to retract your foreskin to apply the cream which can be somewhat painful if you're very sensitive.
