I had a cholesteatoma and it was "worked on" over a period of several months. The doctor would drain the fluid and the moment he touched it, the room would spin and I had extreme nausea and urge to vomit. This is why such a long process - he can't do much in one sitting. The alternative was major surgery - not interested in that again. :) as for nerves in the ear - yes, absolutely.
