A patient getting allopurinol 300mg /day,
he does very well more than 2 yrs with no
gouty attack.He then experiences another
episode of right ankle swelling.What to do
now?

First of all, you may like to call your
doctor to ask for his advice. Fyi:
** do not stop taking allopurinol.
It may trigger gout attacks when you
re-start it later.
** treat the current attack just like
another attack. "emergency gout
treatments" section in http://www.Icuredmygout.Org
describes what to do in such situation.
** allopurinol can lower the blood
uric acid level and reduce the frequency
and severity of gout attacks. But it
does not always immune one from having
further attacks. A study shows gout
patients who had been on allopurinol for
months and years still had msu crystals in
their joints. When the crystal coatings
are shed to expose the raw crystals, they
trigger the attacks. You may like to
track down what trigger the attack this
time so you can prevent another attack
later.

After treating the current acute attack
with conventional management , is I
justified to increase the allopurinol dose
(>300mg/day)?

I suggest you talk to your doctor about
allopurinol dosage. Fyi: the typical
allopurinol dosages are:
** mild gout:
200~300 mg, once daily.
** moderate to sever gout: 400~600
mg, once daily.
I would think your gout is mild because
you had only one attack in 2 years.
Baking soda in water is not a conventional
treatment for gout. I just accidentally
found it to be extremely useful. It
seems to have worked well for many other
gout patients, too. Read the warning
and possible side effect described in the
webpage before trying. Good luck.

to zyloprim drug information,
you should:
stop taking this medicine and check with
your doctor immediately if any of the
following side effects occur:
* more common
o skin rash or sores, hives, or
itching
* rare
o black, tarry stools; bleeding
sores on lips; blood in urine or stools;
chills, fever, muscle aches or pains,
nausea, or vomiting—especially if
occurring with or shortly after a skin
rash; difficult or painful urination;
pinpoint red spots on skin; redness,
tenderness, burning, or peeling of skin;
red and/or irritated eyes; red,
thickened, or scaly skin; shortness of
breath, troubled breathing, tightness in
chest, or wheezing; sores, ulcers, or
white spots in mouth or on lips; sore
throat and fever; sudden decrease in
amount of urine; swelling in upper
abdominal (stomach) area; swelling of
face, fingers, feet, or lower legs ;
swollen and/or painful glands; unusual
bleeding or bruising; unusual tiredness
