Multiple Sclerosis Questionnaires

Anyone that would like to help me with my year 12 assignment; the effect of multiple sclerosis on an individual and their family could you please copy and paste these questionnaires to a reply and fill them out for me! I would really appreciate it. There are 3. The first one is for the person with ms. The 2nd is for the family of the person with ms. And the third one is for a friend of the person with ms. Your time and effort is greatly apprieciated. Names do not have to be provided.
Questionnaire 1 for the person with multiple sclerosis

4. Did you think you had ms before you were diagnosed?
5. What did you know about ms before you were diagnosed? (nothing/a little bit/everything)

11. What symptoms do you have?
12. How has ms affected you physically? (mobility)
13. How has ms affected you mentally? (emotions)
14. How has ms affected you socially? (going out)
15. How has ms affected you economically? (not being able to work to earn money/expenses to help your everyday living)
16. How has ms affected you emotionally? (feelings)
17. Has ms affected your everyday living?
18. What treatment have you tried for ms?
19. Have these treatments helped in any ways?
20. Are you joined to any support groups for ms? ( if yes please specify)
21. Has any one in your family had multiple sclerosis before?
22. How has ms affected your family?

Questionnaire 2 for the family member
1. What is your relation to the person with ms? (eg mum, dad, daughter)
2. How long have you known they have had ms? (years or months)
3. How did it make you feel when you found out?
4. How much did you know about the disease? (nothing/some/a large amount)
5. How were you told about it?
6. Who told you they had ms?
7. How much do you know about ms now? (still nothing/some/a large amount)
8. Have you noticed a change in the person with ms? (if yes please specify)
9. Do you notice any of their symptoms that the person with ms has? (if yes please specify)
10. Has it had an effect on your life since that person has been diagnosed with ms?(if yes please specify)
11. Have you changed your house in any way to suit mobility problems? (if yes please specify)
12. Have you helped the person with ms in the following ways:
physically
emotionally
financially
fund raising
researching information
receiving help

10. Whats are they like now after being diagnosed with ms? (happy, cheerful, outgoing, sad, deppresed)
11. Do you find it awkward when they talk about ms?
12. Have you modified your house to help your friend with ms? (if yes please specify)

Please Someone Help Me!!!

I really need help! This assignment is for my year 12 hsc. The mark that I get goes towards my uai!!! So far I only have my grandma and 1 other person! Please please help me! I want at least 5 to help me with my assignment. It is only to see how ms affects the individual and their family! You dont have to fill all of them out if you dont want to! Just please please help me!!!

Hi amanda

just a suggestion. Maybe to make it more anonymous you should set up an e-mail address rather than asking people to post their replies.

This is obviously fairly contentious in some respects so if you do do it discuss it with you parents and maybe a teacher to make sure that you've covered all you basic internet safety issues, ie set up a hotmail address that does not identify you in any way. Do not get involved in any correspondence with people (you just don't know who they are). Never divulge any information about yourself (see rule 2!) however trivial it may seem. Never arrange to meet anyone you don't know.... The list goes on and is quite obvious but I can't make the suggestion without pointing this out.

Anyway as I said this would mean that peoples answers aren't available for all to see, unlike a standard posting and may get you more responses. Unfortunately these websites often take a long time to get a reply but it may be worth a shot. I'd certainly fill it out and send it on for you but make sure you never give out any details about yourself or use your personal e-mail address. Discuss it with your folks/teachers (cause no doubt they'd go nuts if you don't) and let us know.

You are sure asking people to put a lot of effort in to helping you with an assignment! Each of those questionnaires could take an hour to fill out, if people thought about their answers. While many people might agree to contribute to genuine medical or academic research, there is not really a lot of benefit for them to put that much effort in to your behalf.

I would suggest you scale down your demands a bit, and you might get more replies.

Thankyou for your suggestions. But earlier I have given people my email address so they could send it to me there. My email address is damanda 7@hotmail.Com feel free to do the questionnaires. I know they are long but they dont need a long answer. Each question is helping me one way or another. I need a different persons view and a close relative. Im really sorry if people dont like the length but try doing them when you are bored on the net or have nothing else better to do. Im really sorry but I need more people to do it! Otherwise I wont get a good mark in my hsc year!
