Q: Do I Have Gout?

It started about 3 month ago.

The effected area is, at the base [top] of the second smallest toe, it felt like a bee sting, swollen and painful, went to the doctor about on day 3. He thought it was some sort of a bee sting, then I was concerned about that it could be some sort of a lime disease bug and he put me on antibiotic. The swelling came down but a bump was still there and the skin reddish in color.
About 8 days after, I went to my family doctor and he said it is just a secondary inflammation of a previous athletes foot and he gave me a cream to put on 2 times a day. I did this for abut 4 weeks, but I noticed, as soon as I stop using this product, it wanted to come back.
Went back to the walk in clinic and another doctor gave me a anti inflammation liquid to put on.
In the meantime, I have seen my mother in-law, [chinese doctor] and she thought it is gout.
So then I have researched it a bit on the web and I have watched my food intake and drink allot of
water...... I find, it has gotten better, but there is still a small "bump" there slightly hard when you touch it and the skin still reddish but doesn't hurt anymore. I went back to my family doctor and told him, that I don't think it is a secondary inflammation of athletes foot, I think it is gout and if I could have a blood test done to check for level of acid.
He said, he will, but it would not confirm 100% by the blood test. He still thinks it is not gout, something in the tandance he said, wasn't sure exactly what he meant, so he gave me another anti inflammation cream.

What do you think it could be??
I thought, I like to know, and if it comes back again, I know what to do.

I still have to figure out, if I have gout.

How long does it usually take to clear things up to normal, after gout attack?

It's been about 3 month, and still have some redness, but I see improvments every day.

Doctor says, never seen gout on the small toe???
Blood test was negative

If you really want to know whether you have gout, infection, etc., you can ask for a joint fluid test. They have to stick a big needle into the joint to draw fluid for the test. It will be very painful, may not be successful for the small joints, and, depending on how the specimen is being handled, the result cannot be 100% accurate.

Unfortunately, diagnosis of gout is not an exact science. The american rheumatology association's criteria for diagnosis of acute gout are: the presence of urate (msu) crystals in the joint fluid and/or in a tophus, and/or presence of 6 or more of the following 13 criteria based on the clinical, laboratory, and x-ray phenomena. How many combinations are there to have 6 or more out of 13? The answer is: 5,812.

1) more than one attack of acute arthritis (joint inflammation).
2) maximum inflammation developed within 1 day.
3) attack of a single joint.
4) redness over joints.
5) metatarsophalangeal joint (the joint between the second and third bones) of the big toe painful or swollen
6) attack of the the big toe in one foot.
7) attack of heel joint in one foot.
8) tophus, deposits of msu crystals in tissues (proven or suspected).
9) hyperuricemia.
10) asymmetric swelling within a joint on x ray.
11) subcortical cysts without erosions on x ray.
12) msu crystals in joint fluid during attack.
13) joint fluid culture negative for organisms during attack.

It looks like without you on this forum, hardly anybody would get an answer!

My attack, whatever it was??... Skin is still lightly red and the bump, just barely visible, well good news and I hope it will stay this way!
