Q: What Exactly Causes Breakthrough Bleeding?

Whenever I look up any contraceptive it explains why periods may be lighter (less build up of tissue because of no ovulation) but I can never find any information on why breakthrough bleeding or spotting may occur. All it tells me is it may (and certainly has!) occur/ed. So what causes it?

Also, I just started bith control pills this sunday (now monday), after depo. When can I hope for my bleeding to stop?
