We have a patient who suffers of following
disorders after application of 27 mg
diazepam in 2 days (approx 50/50):
-cannot keep things in mind (always forget
nearly all, after a few minutes till a few
hours)
-always headache and swindle
-always tired
-nearly no motivation
-loss of nearly all memory for a period of
approx 2 years before the application of
diazepam (forgot names, technical
knowledge, general knowledge, social
knowledge, language knowledge etc..)
-loss of mental speed, cannot combine
things, cannot remember things etc.
=> is always "blocked" with regard to
"higher" brain functions.
Please don't ask why the diazepam was
applicated, we checked that and that's not
the reason.
The patient was diagnosed with
schizophrenia-form psychosis one year
later after the diazepam application. He
was applicated with nearly all atypical
antipsychotics, but nothing helped it only
got worse with this meds and patient had
nearly all adverse effects, before the
patient read the package insert, which the
psychiatrist has never seen, too.
The more the patient took antipsychotics,
the more the patient symptoms fit to a
schizophrenia-form psychosis.
The patient was added a sexuall
dysfunction with this antipsychotics. He
still suffers from this sexual dysfunction
after not taking antipsychotics for over 6
month. When he stopped taking
antipsychotics the sexual dysfunction
became a little bit better.
I am sorry for possible language and/or
vocabulary mistakes, but English is not my
first language.
I hope someone could gimme a hint what i
have to do to cure this patient.
Thank you!

Why he was brought in your office
(hospital)?
What was his mental state when you saw him
for the first time?
Did he have a history of mental condition
before diazepam application?

Thanks for reply!
he came to us because nobody could help
him before. Actually we have at least an
idea what he is suffering from.
It's a brain metabolism change because of
diazepam. But we don't really know what to
do to cure that.
Mental state at first view:
Well, he looked good, not depressed, made
jokes, laughed hearty.
I talked to some of his friends, without
asking him. And he doesn't twit us.
And i talked to several psychiatrists and
they told me that, if there is a
depression, a depression doesn't cause
such symptoms, from one minute to
another.
history:
Well, he took some antideppressivas a few
month before (but only in real small
dose), because of lovesickness (now i know
this is completly wrong to give anti
deppressivas for lovesickness). But this
was gone few month before diazepam
application.
i hope you can help me further.

he has the symptoms above (at the top of
this page).
no, he takes homeopathy (from a doctor for
western medicine and homeopathy, for more
than 6 month now), with tiny positive
changes. But he is and we're still waiting
for the "healing", because tiny changes
are not the point.

Forgetfulness, loss of memory (anterograde
amnesia), fatigue and a hangover effect,
headache, changes in sex drive, mental
depression, and confusion aren't common
side effects of using diazepam, but are
possible ones. Even less common, but
serious event, is behavior change
(amnestic effects may be associated with
inappropriate behavior)
Numbed emotions. In susceptible patients,
an unnoticed depression may become
evident.
I found this from a medical site:
Substance-induced psychotic disorder
Although substance-induced psychoses
frequently resolve in less than 1 month,
sustained substance-induced psychoses in
abstinent persons may be indistinguishable
from schizophreniform disorder. In the
absence of objective diagnostic criteria,
the distinction is made based on the
extent to which the clinician believes
substances have contributed to the current
symptom profile.
Neuroleptic-resistant psychosis in
patients with schizophreniform disorder
may be managed through approaches similar
to those used in patients with
schizophrenia, including adjustment of the
neuroleptic dose, addition of lithium, or
addition of anticonvulsant agents and
older typical antipsychotics.
Waiting for your post!

Hello, juergen28! Posted: 04-15-08 07:42am
I'm impatient to know what have happened
since the last post.
Did you find my post helpful?
Have you achieved some improvements with
his treatment?

thank you for the post, but it wasn't
helpful.
he got change of sex drive from
antipsychotics. He takes no antipsychotiks
for more than 9 month now and the
dysfunktion is still the same.
The symptoms he suffers from are side
affects of diazepam. not very common but
known. And the sexual dysfunktion are
common side affects of
antipsychotics.Keeping the sexual
dysfunktion for more than 9 month is not
common, but i can explain it with regard
to the active ingredient.
I think it's very strange that he got this
symptoms (except sexual dysfunktion)
approx. 15 -20 minutes after
diazepam-application.
I didn't find useful information until
now.

Have you tried treating him with typical
antipsychotics?
Have you suspected that diazepam just
enhanced some psychological or physical
problem existing prior to ingestion of
this drug (in addition to the many new
problems diazepam has created)?
There is a wide variability in the
patterns of response to benzodiazepines
among individuals, in both therapeutic and
adverse reactions, both wanted and
unwanted.
This variability has a strong impact in
the area of dependency, the probability of
an individual becoming dependent on these
drugs. It can readily explain why some
become dependent in a few weeks (shortest
recorded case - seven days). Here comes
the question about the Post Withdrawal
Syndrome.
For how long he was taking diazepam for
"love sickness"?
Benzodiazepines produce impairment of
cognitive functioning and psychomotor
performance, independently of abuse,
dependency, non-dependency, normal,
healthy, young or old subjects.
The functional brain damage causes
increased morbidity, increased mortality
and social deterioration.
In general much of the impairment is
slowly reversible.
Some aspects show improvement after six
years, some are semi-permanent or
permanent.
Is he aware of his reduced capacity or the
fact that he is not functioning well in
every day life?
Best wishes!

hi,
with the most common atypical
antipsychotics. Ziprasidon, Aripiprazol,
Amisulprid, Olanzapin.
Either he didn't tolerate it or he got no
affects (but sedation) or both.
He told me that the rest of his
brainfunctionality was killed with this
antipsychotics + sexual dysfunktion.
He was applicated with diazepam 2 times
(once a day). with overall approx. 27mg.
Approx. 15-20 minutes later he suffered
from the symptoms in my post at the top.
Yes he is aware of his lost
brain-functionality. He was one of the
best students at graduating class of a
technical top-school until diazepam
application.
He doesn't hear things, see things, feel
things which aren't there. Moreover he
doesn't think that someone wants to damage
him. And this are the main characteristics
of schizophrenia-form psychosis.
And the more the patient took
antipsychotics the more his symptoms fit
into schizophrenia-form psychosis.
Hi Juergen,
I am so sorry to hear about this
situation. I was in a similar one, with a
Rhumatoid Arthritis med that caused me to
have symptoms of bipolar. The same thing
happened to me, where the Drs put me on
all these meds and they all failed because
it was a side effect to the med, and the
Drs didnt catch it for over a year. I
have been off of the R.A med for about 3
months and my mental symptoms are leaving
me. One thing I did was rid myself of ALL
medicines, I take as little over the
counter meds as possible. I did a
"cleansing" with my diet, and have gone
over the top with exercise. Also staying
away from caffenine and anything that can
be a possible stimulant has helped.
Drinking caffieine free teas, herbal based
has been helpful.
Have you considered taking him to any high
profile hospitals to see a specialist,
Thomas Jefferson in P.A is a good one to
try. They helped me.
I wish you the best of luck, hang in
there... the med may just be taking its
time to get out of the system.

I'll just quote part of my previous post,
that "In general much of the impairment is
slowly reversible. Some aspects show
improvement after six years, some are
semi-permanent or permanent."
Wish you a lack with solving this case!
I'll be pleased to share with us all new
moments that will come in treatment of
this patient!
