Q: Cracked Foreskin On Penis

a few months now I have something that feels like cracked skin beneath the head of my penis. i treated it w/ cream for yeast infection. it got better, but then it came back again. so i have been putting the cream on and once i continue applying daily i have no probs, as i stop it comes back about after 1 week. once it comes back it is dry so it hurts when i peel back the foreskin or get a hardon. what can it be?
