Q: Fluid In Ear

I have fluid in my ear.

The doctor said take sudafed, gargle with salt and bear down to help the fluid in my ear be released.

Its done nothing. I'll probably have to go to an ENT but I have no idea the questions to ask or what would potentially happen.

Age: 23
Height: 5'4"
Weight: 128
Diet: no corn syrup, no white flour, little to no refined sugar, light on red meat, almost no pork, lots of greens, lentils, hummus, etc.

Symptoms: stuffy feeling to extreme pressure feeling in right ear, dizzyness upon standing or dancing (formal/ballet/tap) or head movement in sports (kickboxing, soccer), noticeably diminished hearing.

Unusual events: severely abused (potential injury, therefore) ages 5-18.
MMR2 shot at age 20 which caused ringing in my ears so bad I collapsed and had a high fever...around 103, I believe.

The pressure is getting continually worse. It is very bad when I wake up in the morning in particular, diminishes during the day and is noticeable at night. Gargling with salt just made my throat raw and sudafed relieves some pressure but its affects are limited and don't reverse diminished hearing.
