Since when you have Occipital neuralgia?
Did you suffer posterior head trauma?
Do you have chronically contracted muscles in the neck posteriorly?
Have you been checked for osteoarthritis of the upper cervical spine?
Did you doctor refer you for a X-ray, CT scan or MRI of your cervical space?

I guess you don't have structural reason for this pain.
All new informations for treatment of Occipital neuralgia are including Botox treatment as a choice, but I'm not sure if it's approved.
If the pain persist despite occipital nerve blockade, the Botulinum toxin should be considered before destructive nerve root surgical procedures are implemented.
Treatment with Botox can be very effective and usually causes relive for two to four months, but still all people are having different results.
You can read this: PDF] Subject: Botulinum Toxin Policy#: DRUG. 00006 Current Effective Date: 03/09/2007 Status: Reviewed …
Maybe you can consider finding another insurance policy.

The company that makes Botox may be able to help. Some pharma companies have programs that help patients and doctors appeal denials of coverage by insurance companies. You or your doctor can contact them and see if they will help with your appeal. Check out this link
