Q: Painful bowel movement

Hi, I'm a 20 year old male, and for the last 1 or 2 years I've been having a very painful poo maybe once every other month. I haven't had a problem with constipation before, as I eat a high fibre diet, and 99.9% of the time my poo is soft and easy to pass out.

This painful poo comes without warning, but once I start to go, the pain comes in waves as the pressure builds between each time I pass the poo out. If there wasn't this intense pain, the poo would be easy to do because it's soft and offers little resistance. However the pain is very intense, often resulting in agony. The pain feels interior around the lower abdominal area. Once I have finished, the pain is completely gone. I have also never noticed any blood.

I am very worried about this being colon cancer, as I have no idea what else it could be. I am also confuesed by the fact that it only seems to happen once every month or so. Any feedback would be great.

Do you have painful bowel movements frequently? Do you feel need for defecation at that moment? Does the pain go away completely after defecation? Do you find stress, anxiety and certain foods and drinks(chocolate, milk, alcohol) aggravating your condition. Is the pain associate with gas? Do you have bloated abdomen? Where exactly do you feel pain?
I think that your symptoms are due to IBS, irritable bowel syndrome. It's symptoms can be experience in patient with other conditions affecting the GI tract. Have you been diagnosed with GERD, Gastritis , Gall bladder diseases and Inflammatory bowel disease?

I don't have painful bowel movements frequently.

I didn't feel the need for defecation at the moment I wrote that post, or now as a matter of fact. The last time I did was about 3 weeks ago (which in my case is fairly recently).

The pain does go away completely after defecation.

I don't suffer badly from stress or anxiety as far as I know and consume milk, chocolate, alcohol and most other foods without any trouble.

As far as I know, the pain is not associated with gas, and I haven't noticed a bloated abdomen.

To describe the pain is difficult. It is extremely painful and like nothing I have felt before. It comes in numerous waves, starting when I begin to excrete and ending immediately as I finish. The waves of pain are synchronised with the stools I excrete. It is interior and feels in the organs around the lower abdominal or anus.

I very much appreciate your input, and i'm sorry for the delay in my reply. Would you suggest I visit a doctor and does it sound serious?

Hi!
You are not mentioning any blood in your stool and you're not suffering from constipation, so, we can rule out hemorrhoids(internal and external) and anal fissures. Have you noticed some mass in your anal area? Are your recent blood tests good? Do you have some other symptoms not connected to defecation process?
I'll suggest you to visit a doctor and exam you with an anorectoscope. That way, the doctor can see if there is an internal mass(tumor that blocks the passage of your stool). Personally, I don't think you have cancer. I think your problem is functional and not to serious. There is another condition when a sever pain is experienced and there is no obvious cause for it.
You should visit a doctor to ease your mind.

Same Problems
Hi,

interesting post. I have been experiencing the same problems for 10 years! Went to a couple GIs, had a flexible sigmoidoscopy, biopsy and all the results are normal. The cause is still unknown.

The only thing I know is that I started having problems after a raw oysters dinner. I got a very severe diarrhea and ever since I am gaving problems. Once every few week. Sometimes 2 wks, sometimes 2 months in between.

The bowel movements start with a normal (or maybe a bit harder stool) and end up with a very watery stool. Sometimes the bowel movements last 1hr. The come in very painful waves. Once the watery stool is out, all the pain stops. Unfortunately, the abdominal pain is very severe and afterwards I fel very exhausted.

Basically, I gave up on finding the problem given that no Gi has found anything.

Al3x, how are you doing? Got any results or diganosis back?

Hello

I've been diagnosed with a hernia and seem to be suffering from the same symptoms... When I begin to have a bowel movement I experience extreme pain.. It's inside and also hard to explain.. I have to force myself to relax through the pain or I would hold the bowel movement in.. once I'm finished the pain is gone... It sometimes hurts when I sit and put pressure on my bottom.. But way inside.. Does that happen to you? my stools are not hard or runny. They seem soft with no blood..

I have a similar problem, almost identical actually. I would like to know the outcome of your exam al3x. I have little spare cash and currently lack insurance so I do not have the means to visit a doctor myself.

You may need to seek an advice from a doctor. I have the same problem as yours too. I'm taking medication.

Painful bowel movement
Hey Al3x,

I'm curious..the pain you were talking about..would you describe them as a clenching of the stomach/intestines that is so severe it makes you feel like you have to vomit?

That's actually been happening to me a bit over the course of the last 6 months or so. W

What would start as a normal bowel movement turns into a bowel movement that sometimes has contractions of such intensity that it feels like (and once or twice has) made me dry heave like I would vomit.

Stool is almost always soft during the contraction bout as well as regular 'movement' portion.

I've given up now too. No GI doctor even seems to understand the amount of pain I'm in, and they all have this wait and see attitude, but it's been 20 years of painful bowel movements every day. Everything comes back looking normal, yet I'm not able to work because I spend hours a day on the toilet in agony, and have difficulty even finding the opportunity to take my kids to a park or go to the grocery store. And I have a good career too, it's just become too unbearable lately. Next visit I think I am going to discuss getting a stoma or whatever it's called. Like you guys I get these waves of pain and intense burning. It's like a person making my bowels into a balloon animal, but hot and burning too. Most days it's not too painful, just urgent and frequent ( like 12 bowel movements every morning, starting normal and each one getting smaller, but not loose and watery) but every once in a while there's a time where it's so intense and painful if I could speak or get off the toilet I'd call 911. My stools aren't hard either. And in fact, the more 'normal' my stool is (smooth & soft) the more painful the bowel movement is.

Original poster --- have you ever seen the mystery diagnosis episode with hereditary angioedema that affected her bowels? It described me to a T, but when I read up on this disease it doesn't match me at all really.
