For it to occur the woman would have to ovulate twice (this is the same thing that happens with fraternal twins) but then she would have to have sex with two or more men within a few days of ovulation. One man fertilizes one egg and the other man fertilizes the second egg.
There was a case where a man's mistress became pregnant with triplets. I believe he was in England. He had a paternity test done on all three babies and found that he was only the father of one of the babies.
it is EXTREMELY rare but it does happen.
There was another case where a fertility clinic mixup caused one of a woman's eggs to be fertilized by the wrong sperm. She wound up having twins. One was white and the other had been fathered by a black man. Their names are Koen and Tuen Stuart.
