Q: Severe concussed / dizzy / dizziness feeling - brain lesion?
asked by: DoctorQuestion on August 30th, 2007
I got taken to hospital 10 years ago. Had a tingling go through my head, lost all senses/balance, felt all tingly, sweats, vomitted. Had a CT Scan and Lumbar Puncture (unremarkable). Got sent home the next day. Went back and forth to the docs because i never really got better. Had splitting headache from LP for 3 weeks. Lumps came up on my legs and was diagnosed as Panniculitis. These continued periodically for the last 8 years. No cause was found. Usually before the lumps appeared i would have a sever faint/dizzy/poisoned feeling which would persist for at least a couple of months. I have not had the lumps for a couple of years but feel my health has deteriated. At best i feel concussed/dizzy. At worst i have a tingly feeling go through my head then like a "power has been turned off feeling" and lose balance (not spinning). This persists for hours/days and then is followed by months of feeling poisoned/dizzy/concussed to the point where my brain is totally fried. ANY IDEAS PLEASE HELP

"Current medical status: Very ill, have not worked for weeks. Feel poisoned/dizzy/faint/concussed all the time. At best i have periods of just feeling concussed. Had a MRI in 2004 (unremarkable). Got told by hospital i had a major anxiety dissorder (i have never been an anxious person.) I saw a psychologist for several weeks who eventually told me that he did not suspect it was an anxiety dissorder and that i was not an anxious person. Saw a vestibular specialist who suspects it is migraine related (although headache does not really affect me). No meds for this have worked. A Chronic Fatigue Specialist thinks it is Chronic Fatigue (Fatigue is not a concern). I don't know what to do next as i feel i am deteriating and will not be around long. (although i have survived 10yrs so far)

Current medical treatment: Have had Serc/Ciprimil/Aropax/Minax/Endep/Sandomigr aine/Verapamil/Diazepam."

An MRI, CT and lumbar puncture can exclude/confirm some possible organic reasons for your symptoms (tingling, lost of all senses, balance loss, nausea/vomitus, faint, dizziness, confusion…). It is possible that various brain lesions are causing these symptoms. Many types of organic brain lesions (multiple sclerosis, lateral tubular sclerosis, cerebrovascular atrophy…) can be visualized or proven with the use of an MRI/CT/lumbar punition as diagnostic tools. In your case, it seems that a brain lesion was excluded because the tests’ results were negative.

On the other hand, some of the symptoms you report (dizziness, nausea, vomitus) can be due to an inner ear disorder so you should consult an ENT-specialist to follow up.

It's likely that the paniculitis is NOT responsible for the most of your symptoms as a local under-skin inflammation. Migraines can cause some of symptoms (nausea, vomitus, dizziness). Nonetheless, the headache is the dominant symptom in that disorder. A final possibility can be that you are experiencing some type of anxiety disorder (somatoform disorder). Somatoform disorder can imitate almost any real organic disorder.
