It doesn’t seem likely that polycystic ovaries can cause the pain you’re describing. Severe pain in the right upper abdomen and back could be due to a gallbladder disorder (gallstones of inflammation). You can request an abdominal ultrasound and blood analyses to confirm or exclude possible gallbladder disorder. Kidney stones are another possibility that can be also confirmed or excluded by ultrasound. You can consult a internist about your condition and to request the diagnostics.
