Severe Pain Under My Ribs Help I Am Afraid of Doctors

okay so I tried to find some other with symptom like mine, I found a few but age was a major difference. I am having a severe pain underneath my right rib cage. It has bothered me for a few years. The pain normally goes away after a few minutes, but this morning I woke up and couldn't sit up, breathe or cough without the pain becoming unbearable! I have a huge fear of doctors and hospitals because of previous experiences so I am tried to avoid by all means going to the E. R. Because I know they will have to use many needles and I will most likely leave there not knowing a thing because "They don't know what could be wrong!!" Normally the pain subsides and I can't continue my day with no interruptions. Not today! I also think I may have an over-active bladder, could this be part of the pain under my ribs?!? The pain lessens when I shift my body around in bed but only for a moment then its just as bad as before. I have already had my appendix removed, and I don't think it could be my kidneys because the pain doesn't go around to my back it is located in one spot and doesn't dispense elsewhere. Could I try to sleep it off or should i rush in to the E. R. Coughing, talking, laughing. moving, descending and climbing stairs , even shifting my weight to the right side hurts. I would like a few opinions before I decide what the best course of action would be!! Please help!
