Q: Testicular cancer symptoms or anxiety ?

Thank you for giving this a read, as I've been a bit worried for a while about the possibility of having testicular cancer. My health concerns started three months ago, when I realized my groin lymph nodes were larger than usual - not swollen really but present. Soon after the realization I contracted a staph infection, which I went on sulfameth for and was treated relatively quickly. My lymph nodes were still present a month later, as well as my neck lymph nodes (sides, jawline, throat area). After noticing these I went on an anxiety-influenced health spree, checking my body for any possible other symptoms and becoming a cyberchondriac (internet self-diagnosis). I had a dull ache in my lung area, I was fatigued much more often than usual, had less of an appetite and generally didn't feel well. I went to the doctors a couple of times and after some blood work, they told me I was recovering from a mono infection (EBV) which was a relief. Unfortunately my worry and symptoms didn't stop there, the monospot test was negative at the time however my EBV IgG and IgM levels were high, and I was worried that I had lymphoma or something that could have given a false test.
For a while I stopped the worrying and felt fine, then I realized my superclavical collarbone lymph nodes were present too. My doctor didn't seem alarmed and told me often times lymph nodes swell all over the body due to a mono infection, and mine were bilateral, not large enough to be deemed malignant or anything of the sort, and that I'd recover shortly. Now, almost a month later, my lymph nodes are still the same size and I've been feeling some slight testicular pain. A few days ago I went cliff jumping and my testicles were sore after some rough landings, and I've been sexually active since. Unfortunately my worry lingers as does a dull sore feeling in my right testicle (mainly). I'm not sure how hard my testicles usually feel to be honest, but they seem quite firm. I haven't noticed any lumps or anything out of the ordinary other than a very, very small groove that I found a few days ago but can't seem to feel now. Also, my right testicle has always been larger than my left, but the larger testicle is the one with the dull pain.

Should I be worried and are any of my symptoms in line for worry? My doctor tells me my tightness in chest feeling lymph nodes fatigue etc. are symptoms of mono, however my lymph nodes are roughly the same size as before and the slight pain down there keeps reminding me that I need to get a diagnosis. I don't have pain while masturbating or ejaculating and don't have night sweats/fevers/etc. I will be getting an ultrasound next week as well as some bloodwork results I recently took for the Mono.

Please help if you think you have any information that could aid this situation! Thank you.

An ultrasound of your testicles in an appropriate next step, and should help to resolve the question of whether or not you have any mass(es) in your testicles.

If you or your doctor are worried about perisitently enlarged lymph nodes (particularly if they are painless and are continuing to grow larger), then a needle biopsy or excisional biopsy of any such suspicious lymph nodes can be performed to rule-out lymphoma.

As an aside cliff-jumping, or any other rough or contact sport is ill-advised following a bout of mononucleosis, as enlargement of the spleen can occur with this condition, and rupturs of an enlarged spleen can occasionally occur with blunt trauma to the abdomen.

Thank you for the reply doc, thankfully my lymph nodes seem to be the same size if not smaller than they were when I first started having mono. I've experienced a tender, sometimes slightly painful feeling in my groin lymph nodes as well as my neck ones, and also experienced a stiff neck on my right side from what I believe was a hurting lymph node.

My doctor didn't seem to be worried or concerned with the possibility of lymphoma and malignancy when I asked him as he was definite my symptoms were from mono. Another question is, is it likely or possible for my lymph nodes to still be present because of the mono infection? I've read many places that my lymph nodes should go back to the unnoticable size they began in within a month or so, and mine have been present for almost three months. My brother told me the same happened to him when he had mono but for two months. Could this be the case?

Thank you for the warning about physical activities, I've been toning it down a bit and haven't been doing any other contact sports. Also I'm 19 years old if that matters.

As a follow-up post, I thought I'd share that my ultrasound came back completely normal and that my symptoms have been relieved. What a relief! I'm feeling much better now, just tired more often because of the Mono.
