Q: Symptoms indicative of pharyngitis
asked by: DoctorQuestion on July 13th, 2009
Sore throat, one side only (right), for 8 weeks. Particularly achey and painful on swallowing, but general noticible pain all the time.

I have had 2 courses of antibiotics, erythromycin 250mg 3x daily then amoxycillin 500mg 3x daily. They helped initially but sore throat never went completley and has re-occured worse after treatment.

I work in paediatric and adult hospitals as a prosthetic practitioner, and am on my medical placement pre- registration year(2 months to graduation). Im not stressed, loosing weight, nor feaverish, my general health is ok. No other medication apart from contraceptive pill.

If anything i've gained a little weight, but i am notibly incredibly lethargic and tired most of the time, with little energy.

I am a smoker, approx 7 cigarettes a day. Low alcohol consumption at weekends only. My partner shows no symptoms of sore throat, nor illness throughout the 8 weeks or before my symptoms.

Any suggestions?

Given the data you reported (sore throat, particularly aching on swallowing, antibiotics initially helped but not solved completely, work in hospitals as prosthetic practitioner), it seems possible that you might be experiencing pharyngitis (inflammation of the pharynx (throat). The tonsils are often affected because they are very close. If the bacteria multiply too much and the inflammation worsens then pus-filled spots may be found on the tonsils when visually inspected.

The treatment for pharyngitis includes antibiotics. The problem is that sometimes the bacteria might have become resistant to antibiotics. This is more likely when there is frequent use of antibiotics as in hospitals. The hospitals are well known for resistant bacteria that cannot be found otherwise. Therefore, it is highly advised that a swab from the throat and from the nose with antibiogram be taken if at least 10 days have passed from the last antibiotic pill taken (otherwise the antibiotic might interfere with the result by not letting the bacteria multiply once again). The antibiogram would precisely determine what antibiotics the bacteria are susceptible to.

Which one of the antibiotics shown on the antibiogram would be best to use, should be recommended by an ear, nose and throat specialist whom you are advised to visit.
