i am a suger pataint last three years currently level is 145..

It's hard to give advice with the limited information that you provide me. A physician - either an endocrinologist (a specialist for diabetes) or an internal medicine specialist - would best be able to evaluate your overall health and treatment needs.

The blood sugar of 145 mg/dl that you mention especially if taken fasting first thing in the morning is diagnostic for diabetes mellitus (sugar diabetes) and should be evaluated by a physician further.
