Q: Severe anxiety accompanied by depression

I suffered severy anxiety and depression at the onset of menopause..8 years ago.
I was unable to swallow even saliva without great difficulty. I lost 15 pounds in 2 weeks because of this. I had to put a capsule of Zoloft in a cup of yogurt every day and force it down..it would take hours. I still take Zoloft..100mgs daily (used
to be 200mgs.) My doctor says I should stay on Zoloft for the rest of my life. My question: Is this safe? Also I had a neck x-ray that showed my muscles are weak and there are goiters in my throat. This was within the last year. Could this cause some of my problem too? I still have trouble swallowing at times but nothing like it used to be. I'm thinking the Zoloft took care of the initial anxiety but perhaps now the problem is actually somewhat physical. Any advice?
