Q: Sensitivity of scrotum

Hi, I’m really hoping somebody can help me. Please forgive my way of explaining this lot.
Around 7 months ago I developed at first sensitivity on my scrotum, kind of in my groin’s and sometimes really under my scrotum. The area now at mention is about an inch or two under the base of my penis over my testicles on my scrotum, just off centre. It got incredible itchy and for months I lived with the embarrassing itch and tried a variety of creams some prescribed others suggested and even I tried some home concoction using garlic and coconut oil...I was using a variety of mainly fungal type creams.
I then went and saw a doctor and had an HIV test as well as another test that ruled out Syphilis....all clear. The Doctor gave me an antibiotic, I went on a full dose of vitamins and I had a steroid cream to what I recall him suggestion that the cream will lift my scrotum.
It worked...a week and a half later I was delighted with life and had often forgotten about my traumatic past 7 months. A few weeks on after a period of initial sensitiveness I was completely healed. Hardly 2 months later now and I felt the familiar tingle...my scrotum isn’t that itchy but its sensitive (probably from the creams) but is often moist and damp and there is like this swelling that happens over my one testicle but on the scrotum...remember the testicles are fine its the scrotum here on the skin. There is often like a hardened swelling to the one side. Its like a lump, I mean I couple cup it with the palm of my hand. Not huge but irregular and often under closer examination there is a rawness on the outer folds of the swelling.
This isn’t a huge thing I’m trying to describe its more like when a guy gets out of the cold water and his scrotum is hardened...well mine can be described like that but to the one side. Both my testicles are the same size all be the one most often than not hangs low and the other is kind of pulled up into the mass of skin or swelling or however to call it...again nothing massive just a raised area that if you take a hold of it and gently squeeze it kind of dissipates. At other times it looks fine and both testicles hand equally, just red and sensitive like they are now.

Sorry for not having the names of all the meds I have tried, one would think this should be more of a concern...well I am very concerned and hope that somebody out there can help or knows what I should do.

I live in a very remote part of east Africa and popping off to the doctor is like a day drive away. If anybody is suggesting that option for the 3rd time!
