Self-inflicted penis wound

I apologize if this post is offensive, I feel I should give details in case they are relevant...

I masturbated last night. Before I became erect, I put my penis through a belt (through the buckle), pulling it down to the base of penis, at the scrotum. I did not pull it over my scrotum. I then became erect and the buckle functioned as a c*ck ring. I masturbated by humping a blanket pushed up against a chair while often tugging on the belt delivering a painful sensation to my penis. This is normal for me and I have never had any problems.

However this time for a reason I now can't even comprehend I let my masochistic tendencies get the best of me and pulled far far too hard. I quite literally at one point pulled on the belt as hard as I physically could. All in all my erection lasting for almost exactly three hours before I ejaculated. I then removed my penis from the buckle. My penis of course was very sore and had been cut in some areas but I did not examine it closely. About 15 minutes passed.

I then put the buckle back on and put another buckle on as well. I had never put two buckles on at the same time before. I masturbated again, this time holding one belt in each hand and pulling each hand in opposite directions, away from my body. I would say about 5 times I yanked as hard as I was physically able and another 10 yanks were very hard i.e. in hindsight harder than I should have been pulling.

This erection lasted for about 30 minutes before I ejaculated. I took both buckles off. The skin was very irritated, large portions of the base of my penis were scraped, I was bleeding, etc. I did not however closely examine the area. I only cleaned the blood off before going to bed.

When I woke up this morning I examined my penis and I noticed that in addition to the cuts/scrapes, the underside of the base of my penis had become misshapen. There is a large bubble, about the size of a quarter, and at its biggest part it rises about 1/2 inch off the penis. A white bump on the bubble is oozing puss. The flap of skin, I am not sure what it's called, but the flap that goes between the testicles and clearly partitions them, it has become pushed to one side (away from the bubble) before curving back to the center. One of the testicles (on the same side as the bubble) hangs lower than the other. When I urinate, the surface of the bubble becomes faintly wet, almost like it's condensation forming. Judging from the smell of the liquid I'd say it's possible it's urine but it's hard to be sure, as my entire genital area smells putrid and no amount of cleaning seems to change this. Although of course I haven't given it a "hard scrub" but I have showered repeatedly and tried to clean by dabbing the area with a wet soapy cloth. I'm not sure if it's relevant but I have experienced arousal today. I have become somewhat hard but not more than 50% erect.

I would appreciate any response. I know I will just hear "go to a doctor", and I will, but I really want to have some idea of just how bad this is before I do, i.e. will I make a full recovery? Like I said, any response would be appreciated, from those with medical knowledge or from people who have experienced anything similar.

I would also appreciate feedback on how I should go about getting medical help. Should I be going to an emergency room? Should I go to my family doctor to get a referral? What sort of doctor should I asked to be referred to?
