Q: Sciatica nerve pain after surgery

I had a L4 and L5 fusion with 3 disk implants and 7 bone graphs. I did really well from my surgery for 10 months and now the sciatica nerve pain is back and worse then it was before the surgery. My doctors keep telling me that my mylogram is pristine so why do I still have the sciatica pain? It is so bad that I can not walk for more then a few steps before I have to sit down. It starts in my left butocks and skips my leg but settles in my ankle and I have to say I could really just cut my foot off about 3 inchs above my ankle and be happy with that. This pain has taken over my life. I try not to take any meds for it, I do take lyrica for it but it does not help in anyway at all, I feel like I am wasting money on this. My doctors prescribe my hydrocodone 7.5 for pain but this again does not help my sciatica pain, I will say that it does help my back pain. What causes this pain and where do I go from here? I am a very active person and keep fit but the ankle pain is over taking my life......what do I do? Where do I go? How do you get your doctors to understand?

It sounds like you need to get another opinion from another spine surgeon that also does ADR.

Do you have a copy of the written report on your mylogram? Could there be a different interpretation between your doctor and the radiologist reading?

I don't know a whole lot about ADR other than what I have read about them on line, but have read that problems can occur if the wrong size was used or if they are not perfectly line up.

I wonder if either of those are a possibility for you?

I wonder if the term "Pristine" being used by your doctor to describe the Myleogram is only in reference to the ADR and your doctor is ignoring anything else that could be showing.

Did your doctor you BMP with your bone grafts? There have been many reports of excess bone growth caused by BMP.

Could that be a problem and excess bone is pressing on the nerves 10 months post op?

I wonder if it would be a good idea to have an MRI done as well?

In any event, it is time for you to get a second opinion. Whatever the reason for your pain, your present doctor based on your post, doesn't know what is causing it and is just filling you with pain meds, not doing anything further to determine the cause.

Get all your test results, pre and post op, your myelogram results, your operative reports etc. and go see another spine surgeon that does ADR and see what a fresh set of eyes will say.

I've not had any fusion surgery done, just a micro diskectomy on what I believe is the same disk; that was 19 months ago - I still have the very same issues you describe as far as pain goes. Lyrica also did nothing for me and my doctor tells me he can see no reason why I'm experiencing issues still; say's I should really just learn to live with the pain because although there is further disk deterioration since the surgery, there isn't enough cause to go back in.

I've been getting pain block injections from another doctor since December 08 and they help - I can at least function during the day at work - but at night, it's game over. I struggle to sleep probably 3 to 4 nights a week now. Sometimes, I get pain so bad in my left leg it feels like my knee is going to explode - then the cramping starts.

When this all started, the pain was focused in my right leg - within 4 months of the surgery - the pain was back, only now it was in the left leg.

Please feel free to write me if you learn anything new that might help.
