Q: Repronex stimulation and ovulation

As I understand, on a normal cycle my ovaries have 1 1/2 - 2 weeks for the follicles to recede (post LH surge) and then begin to grow again on day 2 of the new cycle. This cycle I had no ovulation and therefore still had 14 large follicles by the time my period came. With no time for the follicles to shrink and disappear what will the implications be for this cycle if in addition to the existing follicles I begin to grow new ones for the new cycle...if we try to conceive this month (via the conventional route) will I still be at risk of multiples as a by-product of our efforts in the last cycle?

And as a side question - My ob-gyn did not know the answer - what triggers the LH surge?

Referring to your description, I assume that your gynecologist was trying to perform “in vitro” fertilization. But stimulation with Repronex (FSH) probably caused too many follicles (14) to start growing. Eventual ovulation of all those 14 ovarian follicles after receiving the HCG-shot would cause ovarian hyper-stimulation syndrome (OHSS) which can be a very dangerous condition so doctors had to stop the procedure.


The ovarian follicles that remained now act like functional follicular cysts. They are producing high doses of estrogen hormone that prevents any new follicle from growing. They act as a natural contraceptive. New ovarian follicle growth and ovulation will be prevented as long as those ovarian follicular cysts are active. Functional ovarian cysts are usually active for a few months. You may not have a new ovulation in the next 2-3 months but you may expect irregular bleeding from the womb to happen.


LH surge, that happens right before ovulation happens, is caused by increased production of GnRH from the hypothalamus and that is probably triggered by the sudden decrease of the estrogen level right before the ovulation.
