Hi, and thanks for your query on eHealth forum!

I am sorry to hear that your brother is suffering since a year. He must have had several investigations done. Usually, such a fever is due to tuberculosis is regions other than lung. Recurrent or chronic fever is seen in typhoid, tuberculosis, hepatitis, SLE, rheumatoid arthritis, Polyarteritis nodosa (PAN) and other auto immune disorders, UTI, drug fever, sinusitis, glandular fever, otitis media, IBS, HIV, STDs, endocarditis, parasitic infestations (like malaria), fungal infections, lymphomas and other cancers etc. It can even be diabetes and hyperthyroidism.

Evening rise in temperature is generally due to anemia, diabetes, hypothyroid states, overwork, stress and fatigue. It is also seen in diseases like tuberculosis. If there are gut issues then GI tuberculosis, parasitic infections, and colitis should be ruled out. Since a confirmed diagnosis cannot be given on net please consult your brother’s doctor and discuss all these possibilities. Investigating these causes can help reach a diagnosis and hence proper treatment. Hope this helps. Take care
