Q: Ramipril + HCT (hydrochlorothiazide) and sunlight sensitivity

I've been taking a 10 mg pill (once daily) of Ramipril (Altace) for over a year, but it's not enough to bring my blood pressure down consistently to something like 135/85.

My doctor is trying me on a combination pill - 10 mg ramipril HCT
(hydrochlorothiazide). Hydrochlorothiazide is basically a diuretic (ie
- water pill).

My pharmacist said that I might (or likely would) experience some
sunlight photosensitivity because of the hydrochlorothiazide (why didn't
my doctor tell me that?).

I've looked up this combination pill and can't figure out if Ramipril
(by itself) would also have made me photosensitive. Skin irritation
upon exposure to hydrochlorothiazide does not seem to be a major side
effect.

So basically my questions are:

a) Will Ramipril HCT make me more photosensitive vs Ramipril only?

b) Why wouldn't trying me on a higher dose of Ramipril be more
sensible vs this combination with a diuretic?

Question A...

I was able to find information that says that Ramipril alone could cause sensitivity to sunlight on drugs-dot-com. Hydrochlorothiazide also cautions against prolonged UV exposure. I am not entirely sure, but maybe the fact that both drugs list this as a possible side effect makes it more likely, but I don't really know... sorry Sad

Question B...

As far as a higher dose of Rampipril goes, ACE inhibitors are largely considered less safe and often even less effective than diuretics. Actually, it is suggested that most doctors usually start their patients on diuretics. A major study was also just published in JAMA which revealed that combination drug therapy was significantly more effective then single. It sounds to me like your doctor is on the right track here! Wink

I've not taken any special precautions regarding sun exposure while on the Ramipril, and have experienced no effects. I've only been on the Ramipril HTZ for a few days, but so far no sun effects.

I would have thought that the diuretic would have caused me to urinate more, but so far absolutely no change in urination. Maybe even a slight decrease?

You are correct in thinking that some people see an increase in urination when they begin taking a diuretic... it just sounds like you are one of the lucky ones who has not!

I would suggest that you keep an eye on the possible decreased urination and any other changes in your body that may correlate to the change in medication and tell you doctor if they persist...

I hate to disagree with you, norx, but everything I read says the photosensitivity comes only from the Hydrochlorothiazide. I checked on five different drug websites, including the site you mentioned, and none of them mention photosensitivity as a side effect.
I do agree that recent studies suggest that using a diuretic is more effective at lowering blood pressure than taking a blood pressure medication alone.

Ramipril Nightmares
I have been having vivid nitemares since I have been on Lupito , Plavix and ramipril. To the extent of sitting up in bed or kicking and screeming , never had then before all different, approx couple times a month. ANyone else with such problems?
