Hello and thank you for posting your medical question on E health forum.

The history provided here indicates that you may be having autonomic neuropathy, which can cause low blood pressures and symptoms like dizziness, lightheaded on sudden changes of posture.

This disorder disrupts signals between the brain and portions of the autonomic nervous system, such as the heart, blood vessels and sweat glands, resulting in decreased or abnormal performance of one or more involuntary body functions.

The causes of this condition include commonly diabetes, infections, neurological conditions (MS, Parkinson's disease), autoimmune diseases, etc.

You may consider to consult a cardiologist and a neurologist who can evaluate your condition and initiate appropriate interventions and decide future treatment measures.

I hope this helps.
