Pressure in arms & chest when i lay down. I have not been able to sleep unless I take a sleeping pill because of this. When I awake my fingers and hands hurt, the joints hurt to bend them. When I am sitting up or standing I don't feel the pressure. I have had back surgery and a hysterectomy in the past three years.

me too!
the same thing happens to me, but i havent had either of the surgeries you've had. i was on birth control (yaz), which gave me all kinds of symptoms (heart & breathing issues, leg pains, etc). i think it may be either hormonal (bc of the hysterectomy if you're on any kind of hormone replacement therapy) or circulatory (which can also be caused by hormones). i'm getting checked out for a possible blot clotting disorder that was either caused or activated by the synthetic hormones (yaz). i'd look into blot clot stuff and possibly get off any hormones if you're on them. hope you get to the bottom of it!
