Q: Post Microdiscectomy pain

I had a lumbar microlaminectomy in December of 07 at L4-L5. I felt almost immediate relief from the surgery but over the next few months a different pain came back and got worse and worse. It started in my lower back but is now centalized mainly in my right buttock with pain going down my right leg. The pain rarely goes away and at times is so severe I can hardly tolerate it. MRI last week showed:

Extensive enhancing scar tissue around L5 nerve root
Possible small right lateral disc herniation at L5-S1
Discs at L4-L5 and L5-S1 are degenerated

1.Is it possible for the scar tissue alone to be causing me this much pain ? Peak 9 on a scale of 1-10.
2. Is another surgery to remove the scar tissue possible ?

I havent seen my neuro since the MRI, I go next week. Any info would be greatly appreciated.

Yes it is possible for scar tissue to be causing all of the pain.

Surgery for removing the scar tissue can be iffy at best. Once scar tissue is formed from surgery, removing it only creates more scar tissue.
But if the scar tissue is from another cause and area, it might be possible to remove some or all of it around the nerve root.

degenerated discs can also be causing the pain as well as a lateral herniation at L5/S1.

You should hear what your spine surgeon has to say and what he/she suggests. if surgery is recommended again for you, I would get at least one other opinion first before undergoing more surgery. I would also ask if the doctor has any idea why so much scar tissue has formed.

Since posting this, I have seen my neuro and it wasn't good. He basically told me that the scar tissue was wrapped arouund my nerve root and he is quite sure this is the cause of my pain. He said that surgery is not an option and has since referred me to he pain is no 90% in my right buttocks. I basically spend all day on the couch, getting up and walking throughout the day. I am unable to return to work. I am going to get a 2nd opinion but I trust what he's telling me. Any thoughts or ideas would be welcomed.

try and relax if you can until you hear what the second opinion spine surgeon has to say. then if he/she offers no different opinion, it will not hurt you to get several more opinions.

I would suggest that you also try getting an opinion with a sports medicine orthopedic surgeon who specializes in spines. They can be more up to date on various new techniques that can help you, but still stay within the mainstream methods of surgery.

have your doctors provided you with any pain meds to help relieve some of the pain?
