Q: Possible Kidney failure

Back in late february, I was taking Xenadrine (without ephedra) and a type of creatine supplement. I was taking the Xenadrine maybe three times a week.. I found myself getting somewhat of a sore throat one night.. The next day I went to the gym to work out, I started feeling a strong pain in my back about a half hour after taking a dose of the xenadrine.. I was drinking about 6-7 32 ounce bottles of water a day (from tap)

A week prior, I felt a pain in my back during the middle of the night, got up, urinated and the pain went away.. So I assumed if I upped my water intake I'd "pee out" the pain.. I was wrong.. My back was hurting still.. I was going to keep lifting, but couldn't as the pain increased with movement.. I woke up the next morning sick as a dog, it was hard to even get into a comfortable position to sleep the night before.. I couldn't find any comfort at all except for sitting in a chair with bad posture.. it was the only relief I could find. I went to the doctor at my university (since I live 6 hours from school) and told him what I was feeling.. He told me I had the flu.. The "sick" feeling went away within a week.. So did the intense pain, but it still felt as if something was there when I lean into that area, as it still does. I came home a week early from school right before spring break to get the matter dealt with. I got blood tests done, KUB Xray to check for stones and when it all came back normal, my doc prescribed me muscle relaxers and anti-inflam. medicine. He told me it was likely muscle related since It started hurting as I was lifting (which it did).. About a month after school started back up, i felt as if my heart was racing one night... So I immediately booked a flight home (scared for my life) that night.. When I woke up in the morning, my pulse was very strong and has been so since.

When I got home the following day I went to my GP... He did EKG, T-wave Test, 24 hr holter monitor and Chest X ray.. All normal. He also sent me for a Testicular ultrasound due to some pain I had in my testicle.. It also came back normal. Eventually I started a terrible bout of anxiety because of the unknown.. I started experiencing symptoms of IBS and such related to anxiety (supposedly).. I thought I had kidney cancer, so eventually I went for a CT Scan, which also came back normal. I returned to school late april (yes I was home for a month) after he assured me it was just back pain and anxiety coupled together which made me feel the way I did..

It's now July, I had a blood work done june 26th and got my results as i returned from a vacation in south carolina.. My uric acid is high at 7.6, creatinine at 1.5, and BUN is at 25 (which is the limit) all other levels were normal, and not even close to high or low.. My EGFR was 58.65.. Now I fear I have kidney failure, based on these results and doctors neglecting my theory. He thinks I was a little dehydrated while taking the test (3 veins collapsed while trying to take my blood, so maybe?) and also was on Flagyl, prescribed by my urologist for what he thought was some kind of infection and having diarrhea the entire week leading up to my test. I've also been to a Urologist 3 times for a discoloration of my penis and my fluctuating levels of semen quality (which seems VERY low as compared to my normal volume). I've also noticed slight petichae on my body (some of which i've noticed years ago), which I heard could be an inflammatory cause of renal failure? Over the past 4 or 5 days, I've been very tired late at night.. I just keep thinking it's my kidney's and everybody keeps pushing me off like I'm crazy or something? Just some extra info- My pain was unilateral, on the left side only.

Yesterday I had a Kidney Ultrasound done- Sonographer said everything was of normal size and he seems to believe nothing is wrong there at all. But told me that he'll schedule me a stress-test to show how well my blood is flowing, so I scheduled it.

My blood pressure is always normal, and I have no problem urinating and no swelling that i can notice. (ankles, feet, wrists, face..) maybe stomach but I got some extra luggage there already, and have gained a couple pounds since i stopped working out on a regular basis. I'm also going to see a nephrologist tomorrow.

My doctor is trying to put me on anti-depressants... As I may be anxious or even a little depressed.. But with good reason.. Doctors haven't explained anything to me, and just keep pushing me off it seems. I just haven't felt like myself since all of this started back in Feb.

A few details about myself:

5-11, 231 pounds, 21 y/o, used to workout 6 days a week prior to all of this.. Still work out occasionally but not with such intensity as before.


I'm sorry this is so long.. And I know you can't diagnose me over the internet.. But nobody seems to know what's wrong.... Me included..

Thank you for anything you can offer.. As I said I will be seeing a nephrologist tomorrow and 24 hr creat. clearance next week.. but any information you have can help me, I've been doing all kinds of internet research and nothing seems to give me any concrete answers. Any things you can offer will help me immensely. Thank you in advance, and again I apologize for this LONG EXTENSIVE question..

Can you be more precise about what exactly would you like doctors to explain to you: your laboratory results or your symptoms?
There are various formulas for calculating EGFR.
According to this formula:

EGRF = (140 - Age) x Mass (in kilograms) / 72 x Serum Creatinine (in mg/dl)

which also consider your weight (which is big), your EGFR is 115 and that is normal. Creatinine level is normally higher in people with more developed muscles and you mentioned that you are doing exercises.
You may need to do some more precise tests for calculating the GFR and estimating the kidneys' function so you should consult a nefrologist.
Did doctors find any proteins or hemoglobin in your urine?

my symptoms is what's worrying me the most.

The petechiae (i took a corticosteroid a few weeks ago for sunburn on my head, and developed a few more this morning and recently ) and semen decrease in particular.


No protein was ever found in my urine to my knowledge, and I don't believe any Hemoglobin was either, as it would probably show on dipstick or machine analysis.. I've had 5 or 6 urine test since i've been having symptoms.. all said to be good. I would imagine today a nephrologist will have me take another?

I'm thinking, as I was drinking heavily that weekend, and working out intensely (twice a day, reducing caloric intake) it maybe had caused pain in my kidney area as well as some form of dehydration, which could've been introduced when I took the xenadrine the day after drinking and should've probably rehydrated before doing so, or not have taken it at all.. would ARF been the case? and in that sense would all my blood tests still come back normal a week after the initial pain?

Because all your medical tests till now were negative I believe that most of your symptoms can be due to anxiety and depression. Heavy drinking can also worse your emotional condition.
You can consult some dermatologist about those petechiae.
You need to do some sperm analyzes (spermogram) to check if there is really something wrong with your sperm.

told me that I was dehydrated when I took the test, I had severe diarrhea for about 4-5 days prior to getting my blood taken.. Also when the blood was taken 3 veins collapsed.

She seems to think I have some muscle inflammation which is what I think was initial kidney pain. But I am scheduled for a 24 hr. urine test tomorrow to see if anything is really wrong.

ultrasound reports
right kidney 9.5 left kidney 9.8... is this normal size?? doc says it's nothing to worry about?!?

I'm reading most men have 11.3...

"MOST men" doesn't mean "ALL men" so I agree with the ultrasound doctor that your kidneys' size is in the normal range.
